  • Marie Curie CHC End of Life Care Fast Track Service is a community based care service. The aim of this service is to respond quickly to support eligible patients and their carers within the community and prevent inappropriate admissions to secondary care. This will be in the patient’s own home.  Ideally you will have recent palliative and end of life care experience.
  • The service works in collaboration with the NHS, social services and providers of end of life care in the community. The successful candidates will be based in West Cornwall.
